ExtenZe Review: Does It Work? Ingredients, Formulas and Where to Buy

An evidence-aware look at what is on the ExtenZe labels, what the research does and does not show, the product's legal and FDA history, and where to buy it.

ExtenZe review: quick summary

  • Category: non-prescription “male enhancement” dietary supplement, not a medicine
  • Formats: Original tablets, Maximum Strength Extended Release gelcaps, ExtenZe Plus (5-tablet card), Testosterone Boost tablets and Cherry liquid shots
  • Key label ingredients: proprietary blends including yohimbe, DHEA, pregnenolone, L-arginine, tribulus and other herbs; blend amounts are disclosed, individual doses are not
  • Evidence status: we found no independent clinical trial of any finished ExtenZe product
  • Safety flags: yohimbe and DHEA carry real cautions; DHEA is banned in drug-tested sport
  • History: a $6 million class-action settlement (2010), a $1.75 million California settlement (2011) and two 2018 FDA notices about “Extenze”-labelled products containing hidden sildenafil
  • Buy from: an official store, BuyExtenze (67-day guarantee) or extenze-usa.com (60-day refund policy)

What is ExtenZe and what does it do?

ExtenZe is a once-daily supplement for men sold without a prescription. The extenze-usa.com FAQ calls it “a dietary supplement for men made with a proprietary blend of ingredients” designed for men 18 and over, and its product pages say the formulas support “vitality, stamina, and overall wellness”. The brand’s most repeated claim is “over 1 billion pills sold”, which describes sales, not results.

BuyExtenze goes further. Its FAQ describes ExtenZe as a supplement that “can increase the size of your erect penis in both length and girth” and says it typically takes eight weeks “to achieve maximum results”. We found no evidence that supports a size claim for any supplement, and similar claims were at the centre of ExtenZe’s past legal settlements (see below). Treat everything under “what does it do” as manufacturer marketing until you have read the evidence section.

If you are looking at ExtenZe because of erection problems, see a doctor first. The NIH’s NIDDK lists heart and blood vessel disease, high blood pressure and diabetes among the common causes, and the NHS advises seeing a GP if the problem keeps happening. Proven prescription treatments exist.

ExtenZe ingredients: what the labels say

Neither official store publishes a full Supplement Facts panel online. extenze-usa.com tells buyers to “refer to the Supplement Facts” on the pack, and BuyExtenze lists ingredients without amounts, stating that “the exact formulation is our proprietary secret”. We therefore used the NIH Dietary Supplement Label Database (DSLD), which records US supplement labels.

ExtenZe The Original tablets

DSLD label 305000 (entered 22 Nov 2023, copyright 2020 Global Product Management, Inc.), per 1 tablet:

  • Folate 400 mcg DFE and zinc 25 mg
  • Male Prohormone Blend, 60 mg: micronized DHEA, pregnenolone
  • Bio-Enhancement Blend, 25 mg: black pepper, white pepper, ginger
  • Male Enhancement Blend, 468 mg: tribulus, maca, cnidium, yohimbe bark extract, L-arginine HCl, epimedium (horny goat weed), muira puama, pumpkin, stinging nettle root, astragalus, fo-ti, hops, boron

An older tablet label (DSLD 276692, copyright 2017) had a 600 mg enhancement blend with Asian ginseng, eleuthero, GABA and damiana, so the formula has changed over time.

ExtenZe Maximum Strength Extended Release gelcaps

DSLD label 304999 (entered 24 Jan 2024) is a “Fast Acting Extended Release Liquid Gelcaps” pack. Per 1 gelcap:

  • Multi Active Blend, 135 mg: yohimbe bark extract, DHEA, GABA, niacin, velvet bean seed, tongkat ali root, tribulus
  • Sexual Response Enhancement Blend, 223 mg: black pepper, cnidium, damiana, DHEA, GABA, Asian ginseng, barrenwort, L-arginine, licorice root, muira puama, pregnenolone, tribulus, folate, yohimbine hydrochloride, astragalus, niacin, tongkat ali, velvet bean, chrysin

That is about 358 mg of blended ingredients per gelcap, including both yohimbe extract and yohimbine hydrochloride.

ExtenZe with Testosterone Boost

DSLD label 328965 lists zinc 25 mg plus a 684 mg blend of yohimbe, L-arginine HCl, L-citrulline, tribulus, micronized DHEA, D-aspartic acid, pregnenolone, ginger, muira puama and fo-ti. The official product page notes that “Testosterone Boost does not contain testosterone.”

ExtenZe Plus and liquid shots

We did not find DSLD labels for the current ExtenZe Plus card or the Cherry liquid shot. extenze-usa.com describes ExtenZe Plus as “a blend of herbal ingredients, amino acids, and nutrients” in a 5-day card with a 1-tablet serving. Check the pack before you take either.

The BuyExtenze ingredient list

BuyExtenze names niacin, folic acid, velvet bean, DHEA, maca, cnidium, zinc, pregnenolone, Piper longum, yohimbe, tribulus, horny goat weed, muira puama, astragalus and L-arginine HCl, and says the product is taken as one gelcap a day. That overlaps with the labels above, but we could not match it to one specific DSLD label, so read the Supplement Facts on the box you receive.

Does ExtenZe work?

We found no independent clinical trial of any finished ExtenZe product, and neither store cites one. extenze-usa.com says only that “individual results may vary”. So whether ExtenZe works is not established.

What the research can say is about single ingredients, at doses that may not match ExtenZe:

  • L-arginine. A 2019 meta-analysis in the Journal of Sexual Medicine pooled 10 randomized trials (540 men with mild to moderate ED) and found improvement with arginine at 1,500 to 5,000 mg a day (Rhim et al., PubMed). ExtenZe’s whole blends are 223 to 684 mg, and arginine is only one part, so each serving holds far less than the studied doses.
  • Yohimbe. The National Center for Complementary and Integrative Health says there is “not enough research to say whether yohimbe as a dietary supplement is helpful for any condition, including erectile dysfunction” (NCCIH).
  • Herbs for sexual enhancement in general. NCCIH’s review of supplements marketed for sexual enhancement finds the evidence for herbs such as tribulus small and inconsistent.
  • DHEA. Harvard Health reports mixed results for sexual function and that DHEA “does not appear to influence the aging process” for strength or physical performance (Harvard Health).

Evidence boundary

Ingredient studies cannot prove that a proprietary blend works, because the amount of each ingredient is hidden and the combination has not been tested. Customer testimonials on sales pages describe experiences but cannot show what caused them.

Does ExtenZe make you bigger?

We found no reliable evidence that any pill permanently increases penis size. Mayo Clinic says of enlargement pills that “none of these products has been proved to work” and that some may be harmful (Mayo Clinic). If size is your main concern, our ProExtender review explains why traction devices, not pills, are the only non-surgical method with published length data, and what that data does and does not show.

Does ExtenZe work like Viagra?

No. Sildenafil (Viagra) is a prescription PDE5 inhibitor. None of the ingredients on the ExtenZe labels we found is a PDE5 inhibitor. If a “herbal” pill acts like Viagra, that is a warning sign, as the FDA notices below show.

ExtenZe side effects and safety

There is no published safety study of the finished product. The cautions come from its ingredients and from the brand’s own labels.

  • Yohimbe and yohimbine. NCCIH says yohimbine “has been associated with cardiac arrhythmia (irregular heartbeat), blood pressure problems, heart attacks, and seizures”, and warns not to use yohimbe with MAO inhibitor or tricyclic antidepressants. It adds that most products do not say how much yohimbine they contain (NCCIH).
  • DHEA. Harvard Health says short-term use can cause acne, upset stomach and mood changes, and that DHEA may lower protective HDL cholesterol and affect estrogen levels.
  • Label warnings. The Original tablet label (DSLD 305000) says not to exceed the serving and lists possible side effects including acne, hair loss, aggressiveness, irritability and increased estrogen. It says to stop and seek medical attention for rapid heartbeat, dizziness or blurred vision. It also contains soy.

Is ExtenZe safe for athletes?

No, if you are drug-tested. DHEA “is prohibited at all times, both in and out of competition” under the WADA list (USADA). BuyExtenze itself notes that professional football and Olympic athletes are banned from taking it.

Who should not take ExtenZe / talk to a doctor first

The label says not to use it under 18, and to consult a doctor first if you have, or have a family history of, prostate cancer or prostate enlargement, diabetes, heart disease, kidney or liver disease, high or low blood pressure or low HDL cholesterol, or if you take antidepressant or antipsychotic medication. We would add:

  • anyone taking nitrates, blood pressure medicine or any prescription drug
  • anyone with anxiety, panic attacks or a seizure history (yohimbine)
  • anyone with a hormone-sensitive condition (DHEA, pregnenolone)
  • drug-tested athletes
  • anyone whose erection problems are new or worsening; get checked first

2010 class action. In December 2010, CBS News reported that ExtenZe settled a false-advertising class action for $6 million. Earlier ads had claimed the product was scientifically proven to increase size.

2011 California settlement. On 21 July 2011, the Orange County District Attorney announced that Biotab Nutraceuticals, Inc. would pay $1.75 million. The allegations included unsubstantiated penile-enlargement advertising, lots containing lead above California’s Proposition 65 limit without a warning, slow or missing refunds, and violating a 2006 injunction over the same claims. Biotab did not admit fault or liability.

2018 FDA notices. On 31 August 2018 the FDA warned consumers not to buy Extenze Plus (lot H061084, exp. 12/2020) or a product labelled Extenze Nutritional Supplement. FDA testing found undeclared sildenafil, which can interact with nitrates and drop blood pressure to dangerous levels. Both were found in international mail shipments. The notices do not say who made them or whether they were counterfeit, which is the practical reason to avoid unknown overseas sellers.

How to take ExtenZe

The Original, Extended Release and Testosterone Boost labels all say to take one serving daily in the morning with an 8 oz glass of water. BuyExtenze says to take one gelcap each day. Do not double up, do not combine formats, and do not take it alongside other yohimbe or DHEA products. The liquid shot has its own limits (see the table). Follow the directions on the pack you actually buy.

Where to buy ExtenZe: Walmart, CVS, Walgreens or online

Official online stores. Two brand-connected stores sell ExtenZe online:

  • BuyExtenze.com (Leading Edge Health). Checked 27 Sept 2026: 1 box (30-day supply) $49, 3 boxes $135, 6 boxes $234, free shipping to the lower 48 states, and a 67-day “Empty Box Guarantee”. You return the unused portion (for multi-box orders, the first two containers plus any unopened ones) within 67 days of receiving your order for a refund of the product price, excluding shipping. Refunds are limited to one order per customer.
  • extenze-usa.com (CB Distributors). Checked the same day: single packs from about $25 to $30 and multi-pack discounts, with a refund policy for orders on that site within 60 days. Its per-pack prices are lower; BuyExtenze offers the longer guarantee window and free shipping.

Walmart, CVS, Walgreens and “near me.” CB Distributors says it supplies convenience stores and distributors, and ExtenZe is widely searched at Walmart, CVS and Walgreens. We did not check store stock or shelf prices, so we cannot tell you what a Walmart price is today. Store purchases follow the store’s own return policy, not either official guarantee. Check the pack is sealed, in date and has an intact blister.

Amazon and marketplaces. We could not verify which third-party listings are genuine. Given the 2018 FDA notices about sildenafil-tainted “Extenze” products from international shipments, avoid unknown sellers.

ExtenZe FAQ

Does Extenze work?

We found no independent clinical trial of any finished ExtenZe product, and the brand says results vary. Some single ingredients, such as L-arginine, have been studied, but at doses far above what ExtenZe’s blends can contain.

What does Extenze do?

It is marketed as a daily male enhancement supplement for vitality, stamina and performance, and BuyExtenze claims larger erections. Those are manufacturer claims. We found no evidence that it increases size, and no trial of the finished product.

Is Extenze safe?

There is no published safety study of the product. Its labels list yohimbe and DHEA, which NCCIH and Harvard Health link to heart rhythm, blood pressure, cholesterol and hormone concerns. Talk to a doctor first, especially if you take medication or have heart problems.

What is the difference between Extenze and Extenze Plus?

ExtenZe Plus is sold as a 5-tablet card and described as a blend of herbal ingredients, amino acids and nutrients. We could not find its current label. The Original and Extended Release products are daily tablets and gelcaps with published label blends. One 2018 FDA notice concerned a lot of an “Extenze Plus” product containing hidden sildenafil.

Can I buy Extenze at Walmart, CVS or Walgreens?

ExtenZe is sold through retail channels and people look for it at Walmart, CVS and Walgreens, but we did not check store stock or prices. Store returns follow the store’s policy. The 67-day guarantee applies to orders from BuyExtenze.

What is the red Extenze pill?

We could not confirm from an official source which ExtenZe product that phrase refers to. Identify any product by the name and Supplement Facts on its sealed pack, and never take loose or unlabelled pills sold as ExtenZe.

Is Extenze FDA approved?

No. Dietary supplements are not approved by the FDA before sale (FDA). The FDA has warned about products labelled Extenze that contained undeclared sildenafil.

What do Extenze reviews on Reddit say?

Forum posts are personal reports. They cannot confirm what was in a pack or whether a change came from the product, so we do not use them as evidence.

Verdict

ExtenZe is easy to find, and BuyExtenze offers a 67-day guarantee on its orders. But the formulas rely on proprietary blends that hide the dose of each ingredient, the labels include yohimbe and DHEA with real safety cautions, and we found no independent trial of any finished ExtenZe product. Its history includes advertising settlements over size claims and FDA notices about sildenafil-tainted products using the name. If erection problems are the reason you are looking, see a doctor first. If you still want to try it, buy from an official store, read the label and check it with your doctor or pharmacist.
