The short answer
ExtenZe is one brand with several separate products, not one pill in different strengths:
- ExtenZe The Original is a once-daily tablet. Its current NIH-recorded label lists folate, zinc and three blends totalling 553 mg, including DHEA, pregnenolone and yohimbe.
- ExtenZe Maximum Strength Extended Release is a once-daily liquid gelcap. Its recorded blends total about 358 mg and list both yohimbe bark extract and yohimbine hydrochloride.
- ExtenZe Plus is sold as a 5-tablet card. We could not find a published Supplement Facts label for it.
- ExtenZe with Testosterone Boost is a once-daily tablet with zinc and a 684 mg blend. Its label says it does not contain testosterone.
- ExtenZe Cherry liquid shot is a 2 oz bottle with its own serving limits and no published label we could find.

ExtenZe The Original tablets
What it is. The tablet most people mean when they search “ExtenZe tablets” or “ExtenZe pills”. DSLD label 305000, “ExtenZe The Original”, was entered on 22 November 2023 and carries a 2020 copyright for Global Product Management, Inc.
Label. Per tablet: folate 400 mcg DFE, zinc 25 mg, then:
- Male Prohormone Blend, 60 mg: micronized DHEA and pregnenolone
- Bio-Enhancement Blend, 25 mg: black pepper, white pepper, ginger
- Male Enhancement Blend, 468 mg: tribulus, maca, cnidium, yohimbe bark extract, L-arginine HCl, epimedium, muira puama, pumpkin, stinging nettle root, astragalus, fo-ti, hops and boron
Original formula vs older labels. If you have an older box, it may differ. DSLD 276692 (2017 copyright) has the same 60 mg and 25 mg blends but a 600 mg enhancement blend that adds Asian ginseng, eleuthero, GABA and damiana and drops maca. Read the panel on the pack you hold.
How to take it. The label says one tablet daily in the morning with an 8 oz glass of water.
Who it may suit. Someone who wants the standard daily tablet with a published label.
Cautions. The label says not to exceed the serving, lists possible side effects including acne, hair loss, aggressiveness, irritability and increased estrogen, and says to stop and seek medical attention for rapid heartbeat, dizziness or blurred vision. It contains soy and is not for anyone under 18.
ExtenZe Maximum Strength Extended Release gelcaps
What it is. A liquid gelcap sold as “Maximum Strength” and “Fast Acting Extended Release”. extenze-usa.com calls it an “extended-release formula” for “vitality, stamina, and overall wellness”.
Label. DSLD label 304999 (entered 24 January 2024, 15-count pack), per gelcap:
- Multi Active Blend, 135 mg: yohimbe bark extract, DHEA, GABA, niacin, velvet bean, tongkat ali, tribulus
- Sexual Response Enhancement Blend, 223 mg: black pepper, cnidium, damiana, DHEA, GABA, Asian ginseng, barrenwort, L-arginine, licorice root, muira puama, pregnenolone, tribulus, folate, yohimbine hydrochloride, astragalus, niacin, tongkat ali, velvet bean and chrysin
“Maximum Strength” does not mean a bigger blend. The gelcap’s blends total about 358 mg, less than the Original tablet’s 553 mg. What is different is the mix: it adds yohimbine hydrochloride, tongkat ali, velvet bean and chrysin.
An earlier Extended Release label (DSLD 252166, 2018 copyright) listed niacin 23 mg, folate 500 mcg and 135 mg plus 221 mg blends with no DHEA, and warned: “Not for use by individuals under the age of 40 years.”
How to take it. The extenze-usa.com product page says one daily in the morning with an 8 oz glass of water. The 304999 database record does not include directions or warnings text, so check the box.
Who it may suit. Someone who prefers a gelcap. The “fast acting” wording is not backed by any published timing data we found.
Cautions. Two yohimbe sources in one serving, with undisclosed amounts. The older gelcap label also lists flushing as a possible side effect.
ExtenZe Plus
What it is. extenze-usa.com sells the “Extenze® Plus 5ct Card” at $5.99 and describes it as “a dietary supplement for men made with a blend of herbal ingredients, amino acids, and nutrients”, in a 5-day supply card with a serving size of 1 tablet.
Label. We searched the DSLD and found no ExtenZe Plus label, and the official product page does not show a Supplement Facts panel in text. So we cannot tell you what is in it, or how it differs chemically from the Original.
How to take it. We did not find directions on the product page. Follow the directions printed on the card.
Who it may suit. Someone who wants a small pack. That is a packaging difference, not evidence of a different effect.
Cautions: the 2018 FDA notice. On 31 August 2018 the FDA warned consumers not to buy or use Extenze Plus, lot H061084, exp. 12/2020. FDA found it “during an examination of international mail shipments” and its testing showed undeclared sildenafil, the active ingredient in Viagra, which may interact with nitrates “and may lower blood pressure to dangerous levels”. The same day, the FDA issued a similar notice for a product labelled Extenze Nutritional Supplement. The notices concerned those specific products found in mail shipments. They do not name a manufacturer and do not say the products were counterfeit. The practical lesson is to buy sealed packs from known sellers and check the lot.
ExtenZe with Testosterone Boost
What it is. A daily tablet whose label calls it “Extra strength” and says “It’s like 2 pills in one!” It also states: “Testosterone boost does not contain testosterone.”
Label. DSLD label 328965 (entered 25 June 2025, 15-count pack), per tablet: zinc 25 mg and a 684 mg Testosterone Boost Proprietary Blend of yohimbe, L-arginine HCl, L-citrulline, tribulus, micronized DHEA, D-aspartic acid, pregnenolone, ginger, muira puama and fo-ti.
How to take it. One tablet daily in the morning with an 8 oz glass of water.
Who it may suit. The label says the ingredients support increased testosterone levels. That is a maker claim, not a measured result. If you think you have low testosterone, ask a doctor for a blood test.
Cautions. Same side-effect warnings as the Original, and the label lists diabetes, liver disease and blood pressure problems among reasons to see a doctor first.
ExtenZe Cherry liquid shot
What it is. A 2 oz cherry-flavoured bottle, $9.99 each on extenze-usa.com, also sold in 12, 36 and 72-bottle cases.
Label. We found no DSLD label and no ingredient list in the product page text.
How to take it. The product page says: “Drink one (1) full bottle. Do not consume more than one (1) bottle within a 4-hour period. Do not consume more than two (2) bottles within a 24-hour period.”
Cautions. Without a published label, you cannot check it for yohimbe or DHEA before buying. Read the bottle, and do not take it with an ExtenZe tablet or gelcap.
What about “ExtenZe Maximum Strength” tablets?
“Maximum Strength” is now printed on the Extended Release gelcaps. An older tablet called “ExtenZe Maximum Strength” is in the DSLD (label 34209, entered 2014) and is marked off market. Unusually, it disclosed some amounts: DHEA 50 mg, pregnenolone 10 mg and yohimbine alkaloids 12.5 mg. Current labels do not disclose these amounts, so do not assume today’s products contain the same.
Which should you choose?
On the evidence, there is no formula to recommend over another, because none has been tested in an independent trial. If you are deciding anyway, these are the real differences:
- Want a published label: The Original (305000), Extended Release (304999) and Testosterone Boost (328965) all have DSLD records. Plus and the liquid shot do not.
- Prefer a gelcap: Extended Release, noting it has two yohimbe sources.
- Want a small trial pack: ExtenZe Plus (5 tablets) or a 15-count box.
- Drug-tested athlete: none. DHEA appears on the current Original, Extended Release and Testosterone Boost labels, and USADA says it is prohibited at all times.
- Erection problems that are new or persistent: see a doctor first. Proven prescription treatments exist.

How long does ExtenZe take to work, and how long does it last?
We found no published data on how fast any ExtenZe product acts or how long an effect lasts. What the sellers say:
- The extenze-usa.com FAQ says “individual results may vary” and does not give a timeline.
- “Fast Acting” and “Extended Release” appear on the gelcap labels as product wording, with no timing figures.
- Our ExtenZe review records that BuyExtenze’s FAQ says it typically takes eight weeks “to achieve maximum results”. That is a maker statement, not a trial result.
- We found no official timing statement for ExtenZe Plus.
The directions are the only firm facts: one serving a day for the tablets and gelcaps, and no more than two liquid shots in 24 hours.
Safety: yohimbe and DHEA
The current Original, Extended Release and Testosterone Boost labels all list yohimbe and DHEA.
- Yohimbe. NCCIH says yohimbine “has been associated with cardiac arrhythmia (irregular heartbeat), blood pressure problems, heart attacks, and seizures”, warns against use with MAO inhibitor or tricyclic antidepressants, and notes most products do not say how much yohimbine they contain. It also says there is not enough research to show yohimbe helps erectile dysfunction.
- DHEA. Harvard Health lists acne, upset stomach and mood changes as mild side effects, says DHEA “might lower protective HDL cholesterol” and can affect estrogen levels, and reports mixed results for sexual function.
Talk to a doctor or pharmacist before taking any ExtenZe product, especially if you take nitrates, blood pressure medicine or antidepressants, or have heart, prostate or hormone-sensitive conditions. Never stack two ExtenZe products.

- BuyExtenze, run by Leading Edge Health. Our review records a 67-day “Empty Box Guarantee” on orders there and that it sells ExtenZe as a one-gelcap-a-day product. We could not load the site on 27 September 2026 to recheck prices, so confirm them at checkout.
- extenze-usa.com, run by CB Distributors, Inc., which says it “is the owner of Extenze dietary supplement products”. It sells every format in this guide, and its refund policy lasts 60 days from purchase.
Retail stores follow their own return policies. Given the 2018 FDA notices, avoid unknown marketplace sellers and loose pills.

What is the difference between ExtenZe and ExtenZe Plus?

Blend size does not measure strength, and individual doses are not disclosed. The gelcap label lists both yohimbe bark extract and yohimbine hydrochloride.
How long does ExtenZe Plus take to work?
We found no official timing statement for ExtenZe Plus and no published study of how fast it acts. The extenze-usa.com FAQ says only that individual results may vary.
How long does ExtenZe last?
No ExtenZe product has published data on how long an effect lasts. “Fast Acting” and “Extended Release” are label wording. The Original, Extended Release and Testosterone Boost directions are one serving daily in the morning, and the liquid shot limits you to one bottle in 4 hours and two in 24 hours.
Does ExtenZe Testosterone Boost contain testosterone?
No. Its label says Testosterone Boost does not contain testosterone. It lists zinc 25 mg and a 684 mg blend that includes yohimbe, L-arginine, L-citrulline, tribulus, DHEA, D-aspartic acid and pregnenolone.
Can I take two ExtenZe pills or mix formulas?
The labels say one serving a day and warn that exceeding the recommended serving may cause serious adverse health effects. Most ExtenZe formulas we checked contain yohimbe and DHEA, so combining them stacks the same ingredients. Do not double up or combine formats.
Was ExtenZe Plus recalled by the FDA?
In August 2018 the FDA warned consumers not to buy Extenze Plus lot H061084 (exp. 12/2020), found in international mail shipments, because it contained undeclared sildenafil. The notice concerned that product found in mail shipments; it did not name a maker or say whether it was counterfeit.
Which ExtenZe formula is best?
None has an independent clinical trial, so no formula can be called best on evidence. The practical differences are format, pack size, blend makeup and how clearly the label is published. Speak to a doctor first, especially about yohimbe and DHEA.
